Your initial appointment will consist of an examination, gum evaluation, and a review of your treatment options. Usually, cleaning can be done or started the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. We allow 2 hours for new patient appointments.

New Patient Forms will also be sent to you through email via one of our platforms (enlive/nexhealth/modento) after you make your appointment. There you inform us of your medical, dental history and review and sign our office policies. We can NOT proceed further with your appointment unless your forms are submitted.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.
Please make sure to state in your forms if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
DO YOU TAKE MY DENTAL INSURANCE?
We make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures.
Our office is a non-restricted dental practice. Many of our patients have dental insurance, and we pride ourselves in helping them maximize their benefits. We work with any plan that allows you the freedom to choose any dentist. Usually, this type of plan is called a PPO plan. If you have any problems or questions, please ask our team. Please make sure to upload your insurance information in detail in your intake forms prior to your visit, we are unable to contact your insurance provider on your behalf unless this information is provided to us prior to your appointment. Please note that providing missing/incorrect information in order to avoid any delays in your reimbursement.
